JOB SUMMARY: The Mental Health Navigator reports to and works under the direction and supervision of the Mental Health Manager, Six Nations Elected Council.
To assist the Mental Health Manager in the performance of statutory, advisory and operational duties related to the successful integration of mental health clients into the community.
To assist the Mental Health Manager and Director of Wellbeing in the provision of culturally appropriate early identification and navigation of young adults to appropriate community services.

BASIC QUALIFICATIONS:
- University degree in social work, psychology or related discipline OR College diploma in social work or child and youth worker or related discipline with 1 year of related experience OR Registered Nurse with a diploma in Nursing with 1 year related experience OR Registered Practical Nurse willing to complete Baccalaureate of Science in Nursing Degree with 2 years related experience
- Must be a licensed member in good standing with appropriate regulatory body
- Must have a vehicle and Class G driver's license
- Must be willing to obtain a Class F driver's license
- Understands the importance of confidentiality and the ability to work with tact and discretion

POSITION TITLE: Mental Health Navigator
REPORTING RELATIONSHIP:
Reports to and works under the direction and supervision of the Mental Health Manager, Six Nations Elected Council.
PURPOSE & SCOPE OF THE POSITION:
To assist the Mental Health Manager in the performance of statutory, advisory and operational duties related to the successful integration of mental health clients into the community.
To assist the Mental Health Manager and Director of Wellbeing in the provision of culturally appropriate early identification and navigation of young adults to appropriate community services.
KEY DUTIES & RESPONSIBILITIES:
Technical Functions:
- Employs appropriate assessment skills in identification and support to young adults
- Employs appropriate mental wellness skills consistent with recovery and harm reduction philosophies from a strengths based approach
- Responds to community crisis with assistant as appropriate
- Provides crisis response coverage during regular work hours
- Provides crisis response coverage on a 24-hour basis on a rotational basis
- Provides opportunities for student employment and student placements with Mental Health by supervising appropriate student placements and summer student employment relevant to position
Communications Functions:
- Employs excellent communication skills to involve appropriate agencies for access to culturally appropriate care as identified by client need, location and consent
- Develop and deliver appropriate programming for young adults
- Coordinate program activities with school administrators, professional staff, and other relevant community agencies including youth programs.
- Utilizes the cognitive behavioral approach to teach skills
- Attends and hosts case conferences with relevant care providers as appropriate
- Attends staff and program meetings as required
Administrative Functions:
- Manages individual client cases in a wholistic and culturally appropriate manner
- Assists in the development and delivery of community education programs and group programming
- Maintains documentation according to organizational policies and professional standards and guidelines
- Gathers and organizes pamphlets, information and referral forms to external services
- Employs effective decision-making skills to prioritize client needs, crisis response and program responsibilities
- Maintains clinic statistics by fulfilling annual work plan activities and documenting of required information
Other Functions:
- Adheres to principles of safety re: body secretion precautions and procedures, correct disposal of bio-medical wastes, management of incidents/accidents/disasters as per protocols in terms of precaution, action, reporting and follow-up
- Performs related duties respective of educational qualifications and skill as may be required by the Director of Wellbeing and/or Mental Health Services
- Provides support to the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Wellbeing
WORKING CONDITIONS:
- Work has a high profile, extensive public contact and is subject to deadlines and interruptions.
- Work may at times be subject to unscheduled hours and out of office sessions.
WORKING RELATIONSHIPS:
With the Mental Health Manager
Receives direction and guidance and discusses plans, prioritizes or interacts to ensure tasks are done efficiently and effectively. Receives instruction and supervision.
With Other Staff
Maintains courtesy, cooperation and teamwork with all staff.
With External Agencies
Represents and promotes Six Nations interests relative to Wellbeing Services; maintains awareness of legislative policy and program changes; seeks to develop collaborative professional working relationships.
With the Public
Represents and promotes Six Nations Wellbeing Services in a courteous, cooperative and positive manner, provides information and assistance.
KNOWLEDGE AND SKILLS:
Minimum Requirements:

- Willingness to maintain ongoing professional competency

Errors in judgement and in the conduct of duties could lead to loss credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Director of Wellbeing, Human Services Committee, Six Nations Elected Council, Government Agencies and the public.
CONTROL:
Guiding principles set by Wellbeing Department and Six Nations Elected Council. Works within the administrative policies and procedures established by the Six Nations Elected Council for the Wellbeing Department and other legislation provided by the respective governments.
