About the Role
Specialized Support Services of Nova Scotia is seeking compassionate and skilled Community Transition Workers to join our team. In this role, you will work alongside care teams to support children, youth, and adults in crisis, who may be experiencing challenges related to behaviors, emotions, and mental health. Services are delivered in community-based settings and may be short-term, intermittent, or medium-term in duration. Our organization provides 24-hour supervision and client care using holistic, behavioral, and systemic approaches.
Key Responsibilities
· Collaborate with team members to implement individualized program plans
· Assist clients with personal care and hygiene routines
· Foster culturally supportive environments that promote client development
· Apply behavior expectations aligned with organizational policies
· Use diverse learning techniques such as modeling, observation, and reinforcement
· Support clients in expressing themselves and engaging in community activities
· Transport clients to appointments and events
· Administer medications and maintain health documentation
· Monitor environments for safety and report concerns
· Maintain detailed records of client progress and daily routines
· De-escalate emotional and physical outbursts effectively
· Participate in ongoing professional development and monthly team meetings
Qualifications/Education & Experience:
Certificate, diploma, or degree in Youth and Child Care, Human Services, Psychology, Continuing Care, or a related field
1–2+ years of experience working with vulnerable individuals in crisis who may experiencing challenges related to behaviors and or mental health
Skills & Abilities:
· Proven ability to build rapport with diverse populations
· Strong understanding of intervention strategies that promote independence
· Experience creating meaningful daily activities and implementing personal protocols
· Familiarity with relevant legislation including the Child Welfare Act and Vulnerable Sector regulations
· Excellent verbal and written communication skills
· Ability to work independently and within a multidisciplinary team
· Initiative to develop new programs and make informed decisions
Requirements:
· Valid driver's license and access to a reliable vehicle
· Criminal Records Check, Vulnerable Sector Check
· Certifications: Emergency First Aid with AED Level C, Non-Violent Crisis Intervention, Mental Health First Aid, Positive Behavior Supports or equivalent
· Physical ability to lift 80+ pounds and stand for extended periods
· Flexible, ability to work weekday and/or weekend days
Job Types: Full-time, Part-time
Pay: From $20.00 per hour
